If you went into the hospital you might have to contact and sign forms from individual Drs to get a complete record. there is usually a fee for this service. I would recommend everyone get a copy of your records, they are yours and can be very useful when going to a new Dr or whatever, it saves a lot of time trying to explain stuff and Drs really appreciate having written copies.
